SET Family Medical Clinics provides Chronic Disease Management for Diabetes, Thyroid & Hypertension. SET offers classes for management of Diabetes & Hypertension. The classes are open to the entire community. The classes are provided to help educate people about the diseases. The classes also give people the information and tools to help manage their diseases. To attend classes or for more information, call SET at 719.776.8850. SET's Chronic Disease Management Program is funded by Kaiser Permanante, Caring for Colorado and Colorado Health Foundation
2010 CLASS SCHEDULE: All Classes are held at 5:30 PM every third Tuesday of the month at the SET Administrative Offices.
